DSM-IV TR Multiaxial Classification and Tool


 

This manual allows for assessment on various axes, which provides information on different domains and assists in planning, interventions and identifying outcomes.

Components:

AXIS I: Clinical Disorder (or focus of clinical attention)

Include diagnostic code/DSM-IV name

 

AXIS II: Personality Disorders/Mental Retardation

Includes diagnostic code/DSM-IV name

 

AXIS III: General Medical Condition

Include ICD-9-CM Codes/names

 

AXIS IV: Psychosocial/Environmental Problems (family, primary group system, social, occupational, educational, health care, legal, crime, others

AXIS V: Global Assessment of Functioning (GAF)
